Online medical questionnaire

Employees who are invited to undergo a medical examination receive a text message with a personalised link to this questionnaire.

Why an online questionnaire?

The medical questionnaire is a mandatory part of the medical examination (Codex I.4, Health monitoring of employees). IDEWE uses a digital tool so this questionnaire can be completed before the actual examination.

This tool is designed to avoid the questionnaire taking too long during the medical examination and potentially running out of time to address your concerns.

This greatly increases the medical examination’s quality and efficiency.

What does the questionnaire involve?

The questionnaire consists of around 20 questions about your health. Have a look at the questionnaire. That way you can see which questions you will be asked in advance.

How does it work?

At least one week before your scheduled appointment, IDEWE sends you a text message with important information on how best to protect your personal data.
Shortly after this message, you will receive a second text message with a link to your personal medical questionnaire, which you can access using your phone.
After authentication, you receive a 6-digit personal access code that will remain valid for 10 minutes. Note: you can use this code only once.
Now you can complete the questionnaire. This should take around 7 minutes.
Do your best to complete the questionnaire in one go. Until the questionnaire has been fully completed, your answers will be saved on your phone. This poses an unnecessary risk. Once you have completed the entire questionnaire, you can submit it and the answers will be removed from your phone.
These answers will guide your discussion with the occupational doctor or nursing expert during your medical examination. After submitting the questionnaire, you will not be able to review or retrieve the answers via the text message link.

Tip!

Use a personal access code (numeric passcode, fingerprint, etc.) to secure your smartphone. This is unrelated to the questionnaire, but always a good idea.

Frequently asked questions

Did you receive the questionnaire on your day off? Don’t worry, you can simply complete the questionnaire at a more convenient time.
Completing the questionnaire will not cost you anything; these costs are borne by IDEWE.
Important: IDEWE will never ask you for your bank account, bank codes or other payment information.

Yes: you can rely on IDEWE to ensure the security of your personal data.

Our connections are secured using up-to-date, commonly accepted security measures. We also undergo regular audits, both by third parties and using our own internal controls. We use these audits to continuously improve our security.

What exactly does our secure process entail?

The following is a detailed explanation of all the steps in the process and what we do to ensure that your data is processed securely and confidentially.

First, you receive a text message with a link to our information page about the online questionnaire. This page contains all the information you need and allows you to have a look at the questionnaire.
Your mobile number is already registered in IDEWE’s system. Either your employer provided us with your number, or you did so yourself during a previous medical examination.
You receive a second text message with a link to the questionnaire.
The link in the text message will always start with https://vragenlijst.idewe.be/xxxxxxx-xxxx-xxxx-xxxx-xxxxxxxxxxxx
When you click on the link, the system automatically sends you a 6-digit access code. Note: This is your personal access code. Do not share it with anyone else.
Your personal access code can only be used once and will remain valid for 10 minutes.
After you log in with the access code, you must verify your identity. Your name and employer are displayed. You enter your date of birth as verification.
The system checks your date of birth. Did you fill in the correct date? You can then start the questionnaire.
Answer all the questions and submit your completed questionnaire. Do your best to complete the questionnaire in one go. The data will not be saved on your phone.
Transmission takes place via an encrypted SSL connection, preventing your data from being read by any unauthorised persons.
IDEWE uses a secure network that is protected against external threats to process your completed questionnaire.
The completed questionnaire is classified as medical data and stored in a medical database.
The answers you provide are subject to medical confidentiality and become part of your personal health file. Only the medical team can access this file.
